ECP Rewards Program Rules

• A program to promote the use of Opticote ophthalmic coatings by retail Eye Care professionals (ECPs).
• Retail ECPs earn Reward Certificates redeemable for rewards (in the form of free coatings or gift certificates) from Opticote. See current promotion for details.
• Only retail ECPs can earn Reward Certificates.
• Reward Certificates are earned by registering Certificates of Authenticity at the ECP Opticote rewards page at www.opticote.com.
• Reward Certificates are redeemable only through participating Opticote customers in good standing.
• Certificates of Authenticity are sent with each eligible coating job to Opticote customers.
• Certificate of Authenticity eligibility is determined by the active promotions listed at the ECP Opticote rewards page at www.opticote.com.
• Opticote laboratory customers are responsible for passing on all Certificates of Authenticity to retail ECPs desiring to participate.

• Will specify the number of registered CAs per physical retail ECP location needed to earn a Reward Certificate.
• Will specify the beginning and end dates for issuing CAs.
• Will specify the Opticote coatings eligible for CAs
• Will specify the coatings eligible for redemption.
• CAs will only be issued to Opticote customers for coatings without special pricing.

• Will be valid (must be registered within) for 30 days from date of coating completion at Opticote.
• May only be registered once.
• May only be registered by active retail ECPs participating in the Opticote Rewards program
• Will only be issued for coatings at regular prices. NC warranty, employee discount, Dr. change and coatings with other price reductions will not be eligible for CAs.

• Must be redeemed at Opticote within 90 days of issue.
• May only be redeemed once per certificate.
• Is redeemable only for a free coating as defined by the promotion specified in the Award Certificate.
• Other Opticote charges may apply (other coatings, shipping & handling, etc…)
• Cannot be redeemed by customers in bad standing or with past due balances.
• Are to the benefit of active registered physical retail ECP locations and will be forwarded to the store owner/manger.

• Opticote reserves the right to change or withdraw the Opticote Rewards program at any time and without prior notice.
• Multiple ECP contacts (dispensing opticians) per location are allowed to register.
• Resisted CAs belong to the store employing the ECP contact at time of registration.
